I decided to put this reference book together to support individuals who are deciding to change an aspect of their lifestyle. I have worked within Health Promotion on the Get Moving Nottingham (Local Action Pilot Scheme) to increase physical activity levels within the most hard to reach groups for four years. After this I took a post as a motivational interviewer on the Nottingham GP Referral Scheme where I have been working for the last four years. The knowledge and expertise I have collated into this reference book can support individuals who are unable to access their health professionals or need guidance to take their first steps to a better quality of life. It can be used as an aid when starting on a GP Referral scheme and to support individual goals. The first part of the book is to increase your knowledge around medical conditions and physical activity, exploring the benefits and costs of making a change, how to set goals and achieve your results, understanding the relationship between your thoughts and behaviors, how to read labels, and what makes a balanced meal. The second part of the book is your ‘Action Plan’ – This can be used to set your personal goals, physical activity, and records you’re eating patterns and monitor your moods. Your action plan can be used with the support of a health professional. Obesity is worldwide epidemic and younger people are being diagnosed with diabetes type 2, strokes, or being diagnosed with coronary heart disease. If you already have one of these conditions then you can still benefit from making positive lifestyle changes and reduce the chance of your condition becoming worse. (more)
